Height and weight

Drives type 2 diabetes harder than anything else here.

Blood pressure

If you don't know, that's a finding in itself. It's the cheapest number to go get.

Alcohol

Family history

Anything a parent or sibling has had. Pick all that apply, and skip it entirely if you're not sure.

Already diagnosed

Anything you've already been told you have. These get moved out of the ranking and into a monitoring list instead.
